Abstract:
NDIS is a challenging and dynamic landscape, requiring a dedicated team to support inpatient NDIS challenges – South Metro Health Service Tertiary Hospital, Fiona Stanley has partnered with Peel Health Campus, a secondary hospital providing NDIS support and establishment of pathways, creating a sustainable and efficient partnership.
The topic:
South Metro Health Service has recently acquired a new hospital, transitioning from private into the public health system. This has required a proactive and intensive support in managing NDIS patients admitted into their site, with a formal agreement established. This partnership has resulted in; positive patient outcomes, improved flow across sites, which establishes a sustainable and financially viable solution.
The issues identified:
– New cohort of NDIS patients admitted, which previously the hospital didn’t directly support or manage
– Limited knowledge and resources to manage NDIS patients who historically are complex, challenging and have increased length of stay.
– Navigating NDIS is challenging, with recent increased complexity due to NDIS legislation changes, requiring consistent oversight.
Strategies implemented:
Formal agreement for Tertiary Hospital NDIS Team to provide in-reach support including:
– Implementation of NDIS processes across the organisation
– Onsite staff NDIS education series
– Direct oversight of NDIS patients admitted, including capturing data and case management support throughout inpatient journey.
– Liaison with collaboration with NDIS HLO to support inpatient NDIS participants.
– Development of resources and dedicated NDIS SharePoint
Conclusion:
NDIS presents many challenges, therefore creating sustainable partnerships between tertiary and secondary hospitals, is mutually beneficial.
