A comprehensive guide to Oral Nutritional Supplements and their IDDSI levels

Abstract


Patients with dysphagia are at an increased risk of malnutrition due to inadequate intake. Research supports the use of Oral Nutritional Supplements (ONS) in this population to manage malnutrition. However, collaboration between Speech Pathologists and Dietitians is essential in selecting appropriate ONS, given many patients with dysphagia require thickened fluids. The International Dysphagia Diet Standardisation Initiative (IDDSI) describes the consistency of food and fluid on an eight-level continuum. This study aimed to increase Speech Pathologists and Dietitians’ knowledge of the consistencies of frequently used ONS through the development of a comprehensive guide featuring IDDSI terminology.
Information on the IDDSI levels of ONS manufactured by Nestle and Abbott Nutrition was obtained from research conducted by the University of Queensland (2019). Syringe testing will be conducted for the remaining ONS per IDDSI guidelines, with repeated trials to ensure reliability. A mixed-methods survey was completed by Speech Pathologists and Dietitians pre- and post-launch of the guide to measure their knowledge as well as confidence and satisfaction.
22 clinicians completed the pre-launch survey. Correct identification of the IDDSI levels of a selection of ONS ranged from 9% to 68%. 43.5% felt ‘dissatisfied’ with currently available resources and 47% reported a lack of confidence in locating information. The guide is expected to be finalised within a fortnight, enabling the timely collection of post-launch data.
The anticipated findings of this study are increased knowledge of the IDDSI levels of frequently used ONS, confidence in applying this to clinical practice, and satisfaction regarding available resources. Areas for future research include the impact of this guide on patient care and safety.
