Abstract

Background
The introduction of the NDIS has provided opportunities and challenges for people with disabilities and dramatically changed the way disability supports are delivered in Australia. The interface between Health and Disability services has always been complex, with the introduction of NDIS contributing to extended hospital admissions and delayed discharges for NDIS eligible patients. A dedicated NDIS linkage team was established at Fiona Stanley Fremantle Hospital Group to support the transition of patients back into the community with NDIS funded supports in place.

Methods
An Allied Health NDIS team was created to support and manage NDIS processes within the hospital, for sustainable discharges. A supportive leadership structure was established to provide governance. The team developed essential resources and training by engaging internal and external stakeholders to facilitate a change in clinical practice. Data was collected, with evaluation of pathways to ensure sustainable practice. Initial primary outcomes were LOS and timely discharge.

Results
There was a reduction in LOS by 20 days, due to streamlined discharge for NDIS participants. This has generated a more positive experience for patients, carers and staff. Comprehensive online resources, regular education sessions, active 1:1 case management support have been established, which are accessed by staff daily.

Discussion
The dedicated NDIS linkage team at FSFHG have developed successful pathways to support patients, carers and staff to navigate NDIS processes. This has facilitated timely discharge and enhanced patient care back into the community. Details of this pathway will be presented.

Elish Kelly is currently working as the NDIS Clinical Team Lead across Fiona Stanley Fremantle Hospital Group. She has worked as a Senior Occupational Therapist across both acute and rehabilitation and has a passion in providing excellent patient care in complex discharge planning.
