Abstract


The rules of public healthcare are usually clear, this is what we will deliver and this is the money you will do it with… (Oh and on or under budget please). Most times when given financial supports to deliver a project in health we immediately think of the extra staff member or additional assets required but how often do we get the chance to Expand our Horizons and think outside of the box. In the Far West (NSW) Local Health District Integrated Care Team we had the opportunity when launching our Planned Care for Better Health model for Chronic Disease, to stop and think about what would make a difference for our team. An issue that was regularly brought up by our clinicians and clients included marketing and explaining what the Planned Care for Better Health program was.
This sparked a journey of creativity how to best get our message out to both audiences about our newly launched program. Trying to break against modelling of traditional chronic disease stereotypes and testimonials we worked with a media company to bring our program outline to life in a short animated form.
The Planned Care for Better Health animation went live in February 2023 with good feedback from clinicians and clients so far. The project piece has since been adopted by the NSW Ministry of Health for use in all Integrated Care Teams across NSW.

Biography

Rebecca is the Manager of Integrated Care for the Far West Local Health District. An Occupational Therapist for the last 17 years with extensive experience in clinical and management roles working in rural and remote parts of South Australia, Queensland and New South Wales.
Growing up in rural NSW, Rural and Remote health, clinical redesign and supporting the future of rural health Allied Health clinicians are just a few areas of interest.
