Abstract


The aim of this project was to reduce falls and falls-related harm by creating a user friendly Mobility Chart tool( Talk the Walk Chart) to prevent falls and promote mobility in an inpatient hospital setting. Recent Severity Assessment Code 1(SAC1) investigations had identified a lack of standardised definitions associated with mobility and function related terminology, insufficient resources for communicating patient mobility and function, ,as well an outdated Mobility Policy. The project included benchmarking across Australia and WA, conducting comprehensive literature review, snapshot audit of existing charts, consumer and staff surveys, pilot and feedback of the new charts. Results included creation of a user friendly chart, that has been well received by clinicians, creation of an E-Learning tool, and creation an updated hospital wide mobility policy at Fiona Stanley Fremantle Hospital Group (FSFHG). Average score for completion of chart and chart usefulness increased significantly, and twice as many consumers were aware of the charts use and function.

Biography

Sandra Dumas is a Senior Physiotherapist, who as special interest in Falls Management in a Hospital setting. Sandra has been involved in setting up an interdisciplinary Falls Pathway for older patients presenting to the Fiona Stanley Emergency Department with a fall, and is a long standing member of the
Fiona Stanley Fremantle Hospital Group Falls committee and the WA Statewide Falls group .
